Jul. 14th, 2004 11:41 pmAlthough the profession of midwifery has re-emerged in the late 20th and early 21st century, the majority of Americans continue to believe that it is unsafe for midwives to perform childbirth. I apply Michael Foucault’s theory that knowledge is the viewpoint of those in power to the currently accepted truth that physicians perform safer births than midwives because they are more knowledgeable about childbirth. Physicians’ powerful status in society enabled them to discredit the profession of midwifery by asserting that midwives’ methods of performing birth were unscientific. Our society’s immense trust in science has caused the majority of people to accept physicians’ declarations that births performed by midwives are unsafe. Foucault’s position that widely accepted truths are sometimes false is supported by the fact that physicians’ incorrect knowledge about midwives’ ability to deliver babies has widely been accepted as truth.

Date: 2004-07-15 05:21 am (UTC)First of all, there are two kinds of midwives in US: the college educated ones and the lay ones. The lay midwives acquire their knowledge through trial and error (sorry, no euphemism for it). As in any other kind of work, there are geniuses (Ina May Gaskin; www.thefarm.org) and then there are others. From what I know, it's the lay midwives that are in danger from the legal point of view, so your friend is safe.
Second, it's a somewhat misconception that midwife assisted birth is safer. The thing is, there is a preselection present: with my medical history, I wouldn't chose a home birth or a birthing center that doesn't have an operating room and a doctor on call. I wouldn't go to a midwife and wouldn't spoil her wonderful statistic of delivering babies with no problem. There is also an incindence reporting issue. Take my friend who gave birth to 3 babies at home and 1 at a hospital (C-section). That time she also attempted a home birth but the baby wasn't coming out and after more than 24 hrs of labor, the [very experienced and knowledgeable] midwife sent her to the emergency room. Guess whose record the C-section went on, the midwife's or the doctor's/hospital's?
In the nineties, the midwife operated birthing centers started popping out everywhere. Most insurance companies covered them as they would cover doctors. The companies providing liability insurance loved them as well, because they thought they would be cheap to cover. But...the centers are closing as rapidly as they opened. Last year Elisabeth Seton birthing center in Manhattan closed because they couldn't afford their liability insurance. The rates were too high. How are the rates determined? Based on actual experience. (I do not imply that the midwives there had too many malpractice incidences; the rates are determined in aggregate, over several years and many centers).
Now, I am not against "natural" birth or midwives. But I am an actuary and believe in data more than emotions.

Date: 2004-07-15 01:31 pm (UTC)I was bombarded by articles saying how giving birth with midwife’s assistance is a “female”, “sharing”, “gentle” way as opposed to dealing with doctors who are “male”, “oppressive”, “patriarchal”. But 99% of my unpleasant experience during pregnancies came from dealing with an incompetent doctor, rude nurses and other hospital personnel, all females.
I also read lots of crap saying how nature designed us to handle birth and that things do take care of themselves. I am very healthy, and yet nature intended for me to miscarry during my first two pregnancies and kick the bucket during the third. I now have two kids to show for the achievements of the obstetrical science.
Overall, I think that the whole business of reproduction is too politicized in US.
